Promotional photos and injury incurred on Columbia County Ballet aka CCB property at 639 Fury's Ferry Rd. Martinez, GA 30907, The North Augusta School of Dance aka NASD at 211 West Ave. North Augusta, SC 29841or any theater/performance area we are in for productions. This applies to CCB and Hip Hop Augusta aka HHA students:
I hereby give permission for HHA, NASD and/or CCB to use any photo that includes my child for promotional purposes. I understand that with the increasing use of social media HHA, NASD and CCB cannot be held responsible for video taken and posted by a parent or friend of a dancer without the knowledge of our administrative staff.

I will not hold HHA, NASD or CCB or any of their staff (paid or volunteer) responsible for any injuries sustained to my child while participating in class or in productions. If I am not available in the event my child needs medical attention, I hereby give permission to the staff of HHA or CCB or any competent medical authority, to render any necessary medical attention to my child.
